FIRST VISIT

 

Prior to your appointment, please do not take pain medication for at least 8 hours prior to your appointment as this may prevent proper diagnosis.  Also please provide the following information at the appointment:

  • The referral form and x-rays that your dentist gave to you (if applicable)

  • A list of your allergies and the medications that you are taking

  • Medical or dental insurance information (if applicable)

       

The initial appointment is usually a consultation during which your medical history, referring dentist’s evaluation, symptoms, diagnosis, and treatment options will be discussed.  Often, pre-treatment of infection is necessary and is explained at this time.  If the procedure is recommended for a separate visit, we will set the appointment and provide instructions for preparing for your endodontic treatment.   

 

Please alert our office staff:

  • If you have a latex allergy 

  • If you have any other medical condition that may be of concern prior to treatment (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) 

  • If you are taking anticoagulant therapy medication (plavix, warfarin, coumadin, etc)

 

We ask that patients who are less than 18 years of age be with a parent or guardian at their endodontic appointments. 

 

FINANCIAL POLICY & INSURANCE

 

We are committed to providing the outstanding care at a reasonable cost.  We accept many insurance plans and will work with you to maximize your insurance coverage on covered procedures, verify your insurance and file your claim. 

 

Insurance policies generally cover only a portion of the total treatment cost and unless arrangements have been made, we ask that you pay your portion of the bill at the time of treatment.  Please call us to discuss our flexible payment options or insurance coverage.

 

For your convenience, we accept cash, check, credit and debit cards in addition to payment plans and CareCredit financing.
